Institutional door-to-balloon times have financial implications, being that they are now linked with reimbursement from the Centers for Medicare and Medicaid Services .7 Although these efforts have already been remarkably successful in reducing nationwide door-to-balloon times, it is not known whether these improvements are connected with an overall decrease in mortality.8 A recently available study from a big regional cardiovascular collaborative did not show that annual mortality reduced among patients undergoing primary PCI, despite huge reductions in door-to-balloon times.9 However, that study was limited by regional results and may have lacked sufficient capacity to identify a survival benefit related to the improved treatment times.
